Wellness Inventory - Membership |

Cost - $40.00
For a $40.00 investment, you will obtain a one year membership in the Wellness Inventory. This will allow you to take the Wellness Inventory twice in a year -- a great way to measure your progress and set additional goals!
For an added $40.00, you can purchase a 1 hour coaching session with Dr Betty who will help you review your wellness inventory and assist you in forming a plan to achieve a higher state of wellness.
The one hour coaching session is listed below and is purchased separately.
Additional counseling can be scheduled for 60 minute sessions in the office, on the phone or by computer using Skype at a time convenient for both you and Dr Betty. Go below and purchase any further Wellness Coaching that you would like to schedule.
Once you have purchased your Wellness Inventory Membership you will receive an email within 24 hours that will give you the directions to begin your program.

1 Hour Coaching sessions can be scheduled as an inoffice visit, by telephone or via Skype.
Dr. Betty will contact you to schedule your appointment.
Coaching sessions are scheduled for a one hour time period.
Fee responsibilities for scheduled coaching sessions are as follows:
Client is responsibile for any additional fees for their use of telephone or Skype computer services. Therefore client must initiate scheduled sessions. Client sessions must be paid for in advance - secured credit card processing is available.
All sessions are completely confidential and will be set up according to an agreed time by both Dr. Betty and Client.
Prior to your first counseling session, Dr. Betty will require you to complete her Counseling Intake Form. Please fill this form out and submit it to Dr. Betty, or you can download the Counceling Intake Form and mail or fax the form if you would prefer.
